Group or Individual Insurance
We attempt to take out as much of the "insurance hassle" as possible for you. We will happily telephone your insurance company to verify your benefits. Please remember that the benefits quoted to us by your insurance company are not a guarantee of payment. We will file your claim. If your insurance company pays the benefits they quoted, the only services you will be responsible for are any non-covered services, deductibles or co-pays. If you have any questions at all regarding your financial responsibilities at our office, please do not hesitate to ask.
“On the Job" Injury (Worker’s Compensation)
If you are injured on the job, your care should be paid for under your employer’s Worker’s Compensation insurance. You will need to report the injury to your employer and verify we are a panel physician before calling our office to schedule an appointment. This office will then contact your employer to verify they are recognizing this to be a work related injury and obtain the name and address of their worker’s compensation insurance carrier. All bills will be sent to that carrier for payment.
Automobile Accidents
Pennsylvania is a no-fault state. Regardless as to who is at fault, all medical bills will be submitted to your automobile insurance carrier. Report the accident to your automobile insurance carrier prior to scheduling an examination in our office. Inform them that you are seeking care at Zabinski Chiropractic. We will submit medical reports and billing to your insurance carrier on a monthly basis.
Medicare
We do not accept assignment from Medicare. Payment is expected at the time services are rendered. Reimbursement by Medicare will be sent directly to you. Medicare will only pay for manipulation of the spine. Medicare pays 80% of the allowable fee once the deductible has been met. Our office completes the claim forms for Medicare at no charge.
Secondary Insurance
Please inform us of any secondary insurance you may have. We will assist you if you need help in filing.
